Background and Objective: Surgery is considered the gold standard for managing hypospadias. However, various surgical methods have been introduced over time. To efficiently compare the outcomes of each method, a generalized assessment tool is essential. Numerous questionnaires have been developed for this purpose, among which the Hypospadias Objective Scoring Evaluation (HOSE) questionnaire is one the most widely used worldwide. However, this questionnaire has not yet been translated into Persian. Therefore, the present study aimed to translate the HOSE questionnaire into Persian and evaluate its reliability and validity for future applications.
Materials and Methods: First, the HOSE questionnaire was translated into Persian using the Backward-Forward translation method by two urologists and an English language expert familiar with medical terminology. It was then translated back into English. Next, the questionnaire was sent to experts and compared with the original questionnaire. The quality of translation and content were finalized and corrected. Moreover, the reliability-validity of the final version, including images after hypospadias surgery of 42 patients, were delivered to eight specialists in pediatrics, urology, and pediatric surgery departments to assess and rate. The collected data were analyzed using SPSS software (version 23) through the test-retest method. The Intra-class Correlation Coefficient (ICC) index and its confidence interval were calculated for each variable and the entire questionnaire. To determine internal validity and reliability, Cronbach's alpha coefficient was calculated for each variable as well as for the entire questionnaire.
Results: The patients included a wide spectrum of hypospadias types, including distal, mid-shaft, and proximal forms. Postoperative images were obtained under uniform conditions with consistent lighting and quality. The content validity of the translated questionnaire was confirmed, with a Cronbach’s alpha coefficient of 0.95 and an ICC of 0.94. The internal consistency of the Persian version of the HOSE score was excellent, as indicated by the high Cronbach’s alpha value.
Conclusion: The proper validity and reliability of the Persian HOSE questionnaire confirms its potential as a standard objective measure to evaluate the appearance outcomes of hypospadias surgery.
