Background and Objective: Tearing of the tunica albuginea in the male penis is a relatively uncommon condition that often causes significant distress and embarrassment, leading to delays in seeking medical attention. This can result in sexual and voiding dysfunction. The critical role of timely surgical intervention (ideally within the first hour post-injury) in the prevention and/or reduction of mid and long-term complications of penile fracture has been previously emphasized. The present study aimed to assess the factors affecting the success rate of penile fracture surgery in preventing penile curvature, impotence, and penile plaques in patients treated at Hashminejad Hospital from 2015 to 2019.
Materials and Methods: The study included all patients admitted to the Urology Department of Hashminejad Hospital in Tehran, Iran, with penile fracture complaints who subsequently underwent surgical repair between April 2015 and April 2019. Medical records and surgical notes were reviewed retrospectively to analyze demographic characteristics, injury mechanisms, symptoms, findings of clinical examination, and location and size of the tunica albuginea laceration. All procedures were conducted and documented by a urological surgeon. Six months post-surgery, patients were examined as outpatients, and follow-up included phone calls and a revisit. Assessments included the IIEF-5 scoring system for evaluating post-injury sexual function and the presence of penile curvature, plaques, or nodules. Data were collected from patient files, follow-up telephone calls, and in-person visits.
Results: The mean age of 90 participants was 35.2±9.7 years. The mean time from penile fracture to surgery was 24±17.6 hours, and the mean size of the tunica albuginea tear was 12.4±9.6 mm. Out of 90 patients, 56 (62.2%) cases reported manipulation-related mechanisms, such as masturbation; 82 (91.1%) subjects had tunica albuginea fracture, and 38 (48.7%) cases had a proximal penile fracture. Urinary tract involvement was observed in 7 (8.7%) patients. The most common clinical symptoms were hearing a popping sound (7.86%) and rapid detumescence (1.81%). The mean questionnaire score was 6.19±3.5. Complications were noted in 29 (32.2%) patients, with 20 (22.2%) developing penile curvature, 16 (17.8%) having penile nodules, and 24 (26.7%) experiencing moderate to severe erectile dysfunction. Age and the size of the tunica albuginea fracture displayed statistically significant relationships with penile curvature, penile plaques or nodules, and moderate to severe sexual dissatisfaction (P<0.05).
Conclusion: The size of the tunica albuginea tear and patient age appeared to be associated with penile curvature, penile plaques, and lower IIEF-5 scores. Manipulation was identified as the most common mechanism of penile fracture, and the most frequently noted clinical findings included hearing a popping sound and experiencing rapid penile detumescence.
