ABSTRACT 
Introduction: Male infertility is a significant challenge in reproductive health, and azoospermia is recognized as one of the most severe forms of male infertility. This study aimed to investigate the pathological results of testicular biopsy samples in infertile men with azoospermia to understand the histopathological patterns and their association with clinical and laboratory factors.
 Materials and Methods: This historical cohort study was conducted on 43 infertile men with azoospermia who visited the urology clinic of Kosar Hospital in Sanandaj during 2021-2022. Testicular biopsy samples were examined using standard histopathological techniques, and data were analyzed using Stata 14 software.
Results: The mean age of the patients was 35 years, with 93% having primary azoospermia and 7% having secondary azoospermia. The most common histopathological pattern was hypospermatogenesis (27.9%) , while the least common was testicular atrophy (14%). The mean Johnson score was 3.14 with a standard deviation of 1.441. 
 Conclusion: Hypospermatogenesis was the most common histopathological pattern in infertile men with azoospermia. The results of this study can contribute to improving the diagnosis and treatment of these patients and serve as a reference for future research in male infertility.
