بررسی اثر تکدوز سیپروفلوکساسین پروفیلاکتیک پیش از سیستویورتروسکوپی ساده بر میزان بروز عفونت ادراری: یک کارآزمایی بالینی تصادفی

<strong>زمینه و هدف</strong>: استفاده بی رویه و نا به جا از انتی بیوتیک ها باعث مقاومت ارگانیسم های پاتوژن و تحمیل هزینه های اضافه به بیماران وسیستم بهداشتی می شود . سیستوسکوپی یکی از شایعترین جراحی های اورولوژی می باشد که بر اساس اکثریت کتابهای مرجع ارولوژی نیاز به انتی بیوتیک ندارد ولی نگرانی مبنی بر عدم وجود شرایط یکسان در ایران لزوم انجام این مطالعه کلینیکی است.<br>
<strong>مواد و روش ها</strong>: در این کارازمایی بالینی ، ۱۶۲ بیمار با سن متوسط ۵۱ سال ( از ۱۲ تا ۸۲ سال) که تحت عمل سیستویورتروسکوپی ساده بدون مانیپولاسیون قرار گرفتند ، به صورت تصادفی به دو گروه تقسیم شدند:گروه مداخله ۱ تا ۲ ساعت قبل از عمل تک دوز سیپروفلوکساسین ۵۰۰ میلیگرمی خوراکی مصرف کردند.گروه شاهد هیچ گونه انتی بیوتیکی دریافت نکردند. تمامی بیماران از نظر بروز علائم تحریکی ادرار ،سوزش ،تکرر ادرار و احساس فوریت ۲۴ تا ۷۲ ساعت بعد از عمل مورد بررسی قرار گرفتند و درهمه افراد ازمایش کشت ادرار انجام شد و با توجه به متغییر های: سن ، جنس ، مصرف سیگار اطلاعات جمع آوری شده انالیز شدند.<br>
<strong>یافته ها</strong>: در گروه با مصرف آنتی بیودتیک در ۲.۴۶ درصد بیماران باکتریوری وجود داشت که این میزان درگروه شاهد ۴.۹۳ درصد بود (<span dir="LTR">p=0.72</span>) . آنالیز چند متغییره در دو گروه نشان داد که باکتریوری با جنس (<span dir="LTR">p=0.12</span>) رابطه نداشت اما در سن بالاتر از ۶۰ سال (<span dir="LTR">p=0.03</span>)و سیگار با باکتریوری رابطه داشت (<span dir="LTR">p=0.04</span>) .<br>
<strong>نتیجه گیری</strong>: میزان بروز عفونت ادراری بعد از عمل سیستویورتروسکوپی ساده بدون استفاده از آنتی بیوتیک پروفیلاکتیک افزایش نمی یابد.<br>
<strong>Introduction:</strong> The assumed necessity of antimicrobial prophylaxis prior to simple cystoscopy is controversial. The aim of this study is to determine whether antibiotic prophylaxis is required before this outpatient procedure.<br>
<strong>Methods: </strong>A total of 162 patients with mean age of 51 years old (range: 12–82 years old) were recruited. They were randomized into two groups: First group did not receive antibiotics (81 patients) and the other group received prophylactic antibiotic before the operation (81 patients with a single dose of ciprofloxacin 500 mg). All patients underwent a diagnostic rigid cystoscopy. 48-72 hours following the operation, urine cultures were taken from all of patients. Further, patients were questioned about newly developed symptoms like dysuria, frequency, urgency and fever after cystoscopy.<br>
<strong>Results:</strong> Bacteriuria was present in 2/46% of the patients in Group that receive antibiotic, compared to 4/93% in Group that did not receive antibiotic (P = 0/27). Multivariate analysis in two groups showed that positive bacteriuria was not associated with gender (P =0/12), but there was association for older than 60 years age (P = 0/03) as well as cigarette smoking (P = 0/04).<br>
<strong>Conclusion:</strong> The incidence of the urinary tract infection is not increased without using prophylactic antibiotics.<br>
